review THE MOSES H. CONE MEMORIAL HOSPITAL Hospital Opens Chronic Disease Clinic Moses Cone Hospital's new Comprehensive Health Care Program (CHCP), operating out of the Internal Medicine Clinic, offers a team approach to health care for persons with certain chronic disorders. The primary goals of this program are to decrease morbidity and mortality among patients with diabetes, hypertension, coronary heart disease, emphysema or obesity. The program is based on the premise that cost and complications will be reduced among patients with chronic diseases who are motivated to comply with treatment. A system of care with easier access as well as a more personalized type of care will be offered through this program. CHCP's patients benefit from the comprehensive involvement of dietary, social service, nursing and pharmacy professionals from the hospital. Close attention is also given by the team to the special problems often associated with chronic disease, including transportation, compliance, patient education, family involvement and financial problems. A physician is available to each CHCP patient for consultation or care as necessary. However, day to day care is provided by an adult/geriatric nurse practitioner, Angela Staab, who also coordinates the team. William Woodell, a pharmacist, monitors drug therapy and counsels patients about their prescribed medications. The dietitian member of the team is Georgeanna Koutsky, who is also in charge of quality assurance of the hospital's Food Service Department. The social worker, Sharon McGrath, is available to assist patients with the emotional components of illness and to identify sources of stress/psychosocial factors which impede health care maintenance. Charles Hansen serves as a clinical research associate to the team. He is Georgeanna Koutsky, William Woodell, Kathy D'Achille, Dr. Stewart Rogers and Angela Staab in conference. helping to collect information which will aid the team in evaluating and improving their patient care sevices. Kathy D'Achille, Pharm.D., a graduate student at UNC-G, is helping to develop the program. Through mutual referral, teaching and quality of care audit, the program will enhance the educational experience of the resident physicians and the teaching staff as well as hospital nursing and allied health staff. The cost effective design of the program will be mutually beneficial to the hospital and CHCP's patients alike. Lucy Reynolds retired in March from her job in Financial Services following 25 years of employment at Moses Cone Hospital.
