Grant Report Form
As noted in the terms and conditions of the grant application form, the Guilford Green Foundation requires that grant recipients agree to provide a year end accounting of the funds use for their granted purpose. For your convenience, we are providing you with this Grant Report Form to be completed for each grant awarded. If a program/project has not been completed, grant recipients must submit a Grant Report on progress to date for review by the Guilford Green Foundation. As a general guideline, grants will not be awarded to organizations that have not submitted a Grant Report for a prior program/project. All grant evaluation reports reflecting progress to date must be submitted no later than July 29th, 2008.
Grant Report Forms may be submitted via e-mail by e-mailing cthompson@ggfnc.org. If submitting by mail, send to: Gifting Committee, Guilford Green Foundation, 301 South Elm St., Ste 211 Greensboro, NC 27401.
Grant Evaluation Reports should follow the format indicated below. (It is acceptable to copy the content below for use in creating a document.) A Report should refer to the original grant Request/Application.

3. Program/Project Title: Board member attendance at the 2007 PFLAG Conference in Washington, D.C.

PFLAG has an excellent education track at the conference. This was an excellent training for our board members.
5. Original Focus/Goals/Objectives of the Program and Actual Results. If results vary from the original Focus/Goals/objectives, please explain and comment.

7. Was the projected number of children to be impacted or affected and the demographic mix achieved? If not, why?
Not applicable. This was not a program for children.

The total budget for attendance was $6,414.47. This included participant’s expenses for food and transportation.
The budget for expenses to be paid by GSAFE was $4,618.07.
9. What were the corresponding actual financial results?

11. Please provide a brief narrative commentary on the overall success or lack of success of the Program/Project.
Attendance at the conference was a great board development opportunity for us. Attending and discussing the workshops was great and it both gave us new ideas and affirmed that much of what we are already doing is on-target.
12. Please list and attach examples of news coverage or other materials that communicated information about the Program/Project to various audiences in the community (also include any materials that acknowledged Guilford Green Foundation’s grant in support of the Program/Project).
