THE IMPORTANCE OF A PROJECTED DISCHARGE DATE 1) Gives the nursing home adequate time to prepare for the patient's admission or return to the facility. (Many nursing homes do not admit patients on the weekends.) 2) Allows Hospice to schedule family conferences and arrange for the care. 3) Allows Discharge Planning RN's and Social Workers to arrange for home health care/equipment in a timely manner. 4) Enables family to arrange/prepare for the patient to return home, including transportation and make the transition from acute care to home a smooth one. 5) Gives ancillary staff a time period to complete teachings, etc. 6) Assists Utilization Review RN's in monitoring the utilization of services. 7) Eliminates unnecessary discharge delays. If you have any questions please feel free to contact the Department of Social Work/UR at 854-6295. Thank you. HOLIDAY ACTIVITIES Santa Claus is Coming to Town!!
